In June 2023, Color and the American Cancer Society (ACS) introduced a cancer screening and prevention program for employers and labor organizations, combining the ACS's expertise in cancer screening, prevention, and risk reduction with Color's leadership in population-scale healthcare delivery. Since announcing our partnership with the American Cancer Society, our program has expanded to support diagnosis management, cancer treatment and care, and survivorship. Through our multidisciplinary Virtual Cancer Clinic, Color is helping employees and members diagnose cancer at an earlier, more treatable stage; caring for patients going through their diagnostic journeys with empathy and speed; helping cancer survivors navigate return-to-work challenges and complex clinical needs; and closing critical cancer screening gaps for high-burden cancers.
We are seeking a compassionate, experienced Nurse Practitioner to join our Cancer Survivorship Clinic, providing longitudinal, holistic care for cancer survivors in collaboration with our oncology team. Services will be delivered remotely. The ideal candidate will be passionate about reducing the impact of cancer on populations, adept at using telehealth technology, and experienced working with patients with a previous diagnosis of cancer. You deeply understand the disruptive impact cancer has on patients' lives, and you are passionate about helping address it.

How You'll Contribute:
  • Conduct comprehensive survivorship assessments, including review of treatment history, potential late effects, and individualized risk factors.
  • Develop and implement personalized survivorship care plans, including ongoing screening, surveillance, and preventive health recommendations based on national guidelines (e.g., NCCN, ASCO).
  • Support patients in managing chronic or late effects of cancer treatment, including fatigue, pain, lymphedema, cognitive changes, or endocrine disorders, in collaboration with oncology and primary care teams.
  • Provide education and counseling to patients regarding lifestyle modifications, wellness, and risk reduction to promote long-term health and quality of life.
  • Coordinate care between oncology, primary care, mental health, rehabilitation, and other specialty providers to ensure seamless continuity of care.
  • Telemedically monitor patient progress and manage follow-up care via video, phone, and messaging to ensure ongoing engagement and adherence to care plans.
  • Order and interpret laboratory tests, imaging, and surveillance studies as appropriate under medical supervision.
  • Prescribe medications and supportive therapies within scope and according to established protocols.
  • Document clinical encounters, assessments, and care plans accurately in the electronic health record, adhering to Color's clinical policies and workflows.
  • Practice under standardized procedures established for the NP role consistent with Color's policies, and in consultation with supervising oncologist/s as dictated by patient need.
Our Ideal Candidate Will Have:
  • A friendly, empathetic, compassionate, Nurse Practitioner who is passionate about treating cancer survivors, helping them to navigate care, and improving long-term outcomes.
  • Masters Degree in Nursing and completion of a Masters Program in Adult, Family Practice, or Oncology-focused NP program.
  • The following current licensure and certification: Nurse Practitioner Certificate; Registered Nurse License; National NP Certification (ANCC or AANP.)
  • Minimum of 5 years of clinical practice experience in an outpatient primary care, oncology, or survivorship setting. Experience with late-effect management, cancer risk assessment, or long-term survivorship care is highly preferred.
  • Minimum of 2-year telemedicine experience preferred.
  • Valid state medical license(s) in good standing - preference for multi-state
  • Strong clinical skills grounded in evidence-based survivorship guidelines (e.g. ASCO, NCCN, ACS) with a high bar for quality, safety, and patient-centered care.
  • Exceptional communication skills with an emphasis on empathy, patient education, and shared decision-making.
  • Team-oriented approach; effective collaborator with multi-disciplinary team including physicians, nurses, genetic counselors, care coordinators, and non-clinical operations staff.
  • Excellent organizational skills with the ability to multitask and thrive in fast-paced environments.
  • Comfort with technology, including EHRs, telehealth platforms, and asynchronous messaging.
  • Experience in the medical technology sector a plus
  • Medical Spanish proficiency a major plus

Base Salary Range: The actual base pay is dependent upon many factors, such as: work experience, market data, skills, geographic location, and business need. The base pay range is subject to change and may be modified in the future. This role may also be eligible for bonus, equity, and benefits.
