Summary:


  • Assures the completion of incomplete medical records within the period as specified by medical staff bylaws. Is able to work cooperatively with medical staff and other healthcare professionals in obtaining documentation to complete medical records. Maintains the delinquent chart count at fewer than 50% of average monthly discharges. Reviews records against completion requirements as documented in the Medical Staff Bylaws or external regulatory agencies.

Responsibilities:

1. Record Retrieval: Retrieval of medical records from designated locations in accordance with established assignments and schedules

a. Ensures that all previous day's discharges are received in HIS;

b. Follow up on missing records to ensure that records are received in a timely manner;

2. Record Preparation: Preparation and processing of paper records

a. Removes all staples, paper clips, and/or sticky notes, etc;

b. Repairs forms, as needed, such as torn, wrinkled, and/or folded pages;

c. Stamps illegible documents with a "Poor Quality Original" stamp;

d. Confirmation that patient identifiers are on every page (front and back) including but not limited to, the patient's name, medical record number, and account number by placing patient barcode sticker on paper documents.

3. Record Scanning, Indexing, and Quality Review: Ensures that paper documents are scanned, indexed, and reviewed for completeness:

a. Appropriately utilizes batch cover sheets and separator sheets for batch scanning;

b. Uses scanner(s) proficiently with selecting appropriate settings and feeding paper documentation correctly;

c. Performs a manual quality check during indexing;

d. Verifies that each page is displayed correctly, has appropriate patient identifiers, documents are of good quality, and that documents are grouped correctly;

e. Ensures that documents are correctly named according to the document types;

f. Maintains 100% accuracy rate of scanned images.

4. Record Analysis and Chart Completion: Analyzes records for completeness and accuracy according to hospital policy and procedures.

a. Accurately analyzes all patient record types for required content in accordance with State and Federal regulations and hospital's Medical Staff Bylaws;

b. Assigns and resolves deficiencies for required medical record documentation;

c. Monitor and works assigned work queues;

d. Performs quality reviews of others' scanned documents for accuracy.

5. Release of Information: Handles telephone calls and customer walk-ins with discretion, especially for release of information, always being mindful of patient confidentiality:

a. Maintains hospital requirements, policies, and standards on confidentiality;

b. Responds to incoming faxes as appropriate;

c. Provides information to customers on "My UNC Chart" and assists them with getting set up;

d. Covers front desk area and release of information requests when third party release of information staff is absent or unavailable.

6. Reviews and edits records as completed by physicians to assure that all documentation is complete in accordance with the Medical Staff Bylaws, Rules and Regulations.

a. Maintains an accurate record deficiency system by entering all edits and deletions as needed.

b. Edits all charts within 24 hours of completion by physician.score

7. Maintains Communication:

a. Assists medical staff with questions concerning record completion requirements.

b. Assists with the education of clinical affiliation students.

c. Assists in orienting new employees and new medical staff members.

d. Consults with physician office staff as needed.

e. Readily assists physicians with record completion needs.

f. Provides accurate and timely information regarding incomplete records.

g. Good communication skills are used inter and intra departmentally. Promotes harmony and teamwork among coworkers and others.

8. Provides weekly notification to physicians concerning their incomplete record status. Provides notification to the Department Chairman and/or President of the Medical Staff weekly/monthly, as needed.

a. Notifies physicians of their delinquent or deficient charts on a weekly basis. Notifies physicians prior to placing them in suspension status via letter.

9.Assists with timely billing of records. Maintains a delinquency rate of 50% or less on a monthly basis.

a. Audits and assures that billing is complete no later than 30 days post discharge to provide timely reimbursement and data needed by internal and external customers.

b. Audits incomplete record file to assure that dictation needed for billing and record completion is put on chart in timely manner.

10. Other Duties: Assists with other duties as assigned by manager or director, to include but not limited to:

a. Forwards completed charts to Performance Improvement for medical record review.

b. Performs quality control functions to assure accuracy

c. Completes other duties as assigned with a positive attitude.

d. Appropriately report any system errors or malfunctioning equipment to leadership and/or Information Services Division (ISD);

e. Orders supplies for departmental use on a timely basis;

f. Assists in the orientation and training of new employees and/or the cross-training of co-workers;

a. Provides role specific education to Health Information Technology students;

b. Assists in answering phones and taking accurate messages;

EDUCATIONHigh school diploma or equivalent required. Coursework in medical terminology preferred. A.S. in Health Information Technology preferred.EXPERIENCEPrevious experience in a Health Information Services Department required. Previous Meditech experience preferred.LICENSURE/REGISTRATION/CERTIFICATIONRHIT preferred.OTHER SKILLS AND QUALIFICATIONSDeveloped knowledge of medical terminology. Moderate computer literacy and familiarity with the keyboard. Excel experience preferred. Knowledge of JCAHO, CMS and other external agencies requirement is regarding record completion.
