Summary:We are seeking a highly skilled and dedicated critical care and cardiology clinical pharmacist to join our healthcare team. As a clinical pharmacist, you will play a crucial role in providing comprehensive pharmaceutical care to patients in critical care and inpatient cardiology units. You will collaborate with healthcare professionals to optimize medication therapy, ensure patient safety, and improve health outcomes. Your expertise in medication management and clinical decision-making will contribute to the delivery of high-quality patient-centered care.

Responsibilities:

  1. Review medical charts and assess patients' medication regimens for appropriateness
  2. Develop and implement individualized treatment plans, including dosage adjustments and recommendations for therapy optimization during multi-disciplinary rounds or other forms of communication as applicable
  3. Promote best evidence-based practice through medical literature
  4. Order entry into electronic health record (EHR) by reviewing and interpreting orders to assure accuracy
  5. Respond to pharmacokinetic consults by assessing their appropriateness, developing a plan, and placing orders per institutional policies.
  6. Answer drug information inquiries from providers and nurses
  7. Documentation of all clinical consults in EHR per institutional guidelines
  8. Adverse drug event and medication error reporting per institutional policies
  9. Participate in quality improvement initiatives and medication-related research projects
  10. Participate in development and implementation of institutional clinical guidelines and protocols
  11. Monitor clinical outcomes, evaluate therapeutic interventions, and propose recommendations for process improvement.
  12. Stay updated with the literature relevant to practice area and health care regulations
  13. Attend continuous education sessions provided by our institution; or other local, regional, and national institutions
  14. Attend conferences, seminars, and relevant educational programs to maintain professional competence (optional)
  15. Participate in teaching pharmacy students and interns on rotations
  16. Participate in teaching our PGY-1 and PGY-2 pharmacy residents
  17. Participate in teaching our providers and nurses through various teaching methods as applicable
  18. Adhere to legal and ethical standards related to pharmacy practice
  19. Patient education and counseling on proper medication use, potential side effects, and precautions. Provide clear instructions and address any concerns or questions raised by patients or their caregivers.
Other Information

Education Requirements:
* PharmD or BS Pharmacy Degree

Licensure/Certification Requirements:
* Active pharmacist licensure with the NC Board of Pharmacy or obtainable before initiation of employment. Recent graduates of ACPE-accredited colleges/schools of pharmacy may fill this role while pending initial pharmacist licensure, but are subject to limitations mandated by the NC Board of Pharmacy and must obtain licensure within 6 months of orientation or sooner if required for operations.
Professional Experience Requirements:

Preferred:

  • Completion of PGY-2 residency training in critical care or cardiology, or equivalent experience
  • Board certification in critical care or cardiology pharmacy practice (BCCP and/or BCCCP)
  • ACLS

Knowledge/Skills/and Abilities Requirements:

  • Prior experience as a clinical pharmacist in a similar clinical role is highly desirable.
  • In-depth knowledge of pharmacotherapy, pharmacokinetics, and drug-drug interactions.
  • Strong understanding of clinical guidelines, evidence-based medicine, and pharmaceutical care models.
  • Excellent communication and interpersonal skills to collaborate effectively with healthcare professionals and patients.
  • Proficient computer skills for documentation
  • Attention to detail, critical thinking, and problem-solving abilities in complex clinical situations.
  • Ability to prioritize workload, work independently, and adapt to a fast-paced healthcare environment.
