FTE Salary Conversion Band

$ FTE conversion salary - $60,000

Schedule: 8am-5pm, Monday through Friday

Weekend work may be needed during emergency situations.

Responsibilities:

This position will be remote and will provide care management to assist with coordination of care, including assessing for health care needs, referring to health plan providers, providing education on benefits/expanded benefits, and provide community resources to address social determinants of health needs. The caseload is 100 members. This position will document in an electronic health record and will be required to document outreach notes, complete assessments, and complete a care plan on each member.

Candidate Requirements
Education/Certification	Required: Bachelor's Degree and 2 - 4 years of related experience.	Preferred:

Top 3 must-have hard skills stack-ranked by importance	1	Child welfare experience
2	Experience connecting families/children to local resources for various services including employment, housing, independent living, justice, foster care, etc.
3	Microsoft Office- Outlook, Word, and Excel
	4	PREFERRED: Health plan experience Foster care and/or adoption experience
