Job Summary

Bring your clinical expertise beyond the hospital walls-while still practicing at the top of acute care.

Mass General Brigham's Home Hospital program is redefining how inpatient care is delivered. As a Per Diem Physical Therapist, you'll treat medically complex patients in their homes-providing true inpatient-level care in a dynamic, interdisciplinary environment.

Why This Role Is Different:
This is not traditional home care.
You'll work with patients who meet acute inpatient criteria, collaborating closely with physicians, nurses, and the broader care team as conditions evolve in real time. Think: active medical management, medication changes, ongoing diagnostics, and rapid care plan adjustments-all happening in the home setting.

You'll also be part of a small, highly collaborative PT team (4 full-time PTs) helping to build and grow one of the largest Home Hospital programs in the country.

What We Do
Home Hospital provides acute inpatient-level care in the home, comparable to hospital-based treatment.

Our Patients
Adult patients (18+) who meet national inpatient criteria. Common conditions include:
*Congestive heart failure (CHF)
*COPD
*Complicated urinary tract infections
*Skin and soft tissue infections
*Other acute medical conditions

About the Role
As a Home Hospital PT, you will:
*Perform acute-level PT evaluations and interventions in patients' homes
*Adapt care plans in response to active medical changes
*Collaborate closely with an interdisciplinary care team
*Focus on mobility, home safety, and discharge planning from the home including long term DME recommendations and caregiver training
*Help shape and grow an innovative care model

Schedule & Flexibility
*Per diem role with highly flexible scheduling
*Shift options: 4, 6, 8, or 10 hours
*Coverage: Weekdays and/or weekends
*Hours generally between 8:00 AM - 6:00 PM
*We work around your availability!

Qualifications

  • Licensed Physical Therapist (PT) in Massachusetts required
  • Minimum of 1 year of experience working as a Physical Therapist in a hospital, rehabilitation, or ambulatory setting required
  • BLS preferred
  • Acute care or inpatient experience strongly preferred; candidates with strong home care experience will also be considered
  • Reliable transportation required to travel across North and South Boston regions that Home Hospital supports.

Education

  • Graduate of an accredited Physical Therapy program.
