What You'll Do

• Educate patients and families about home health and hospice services
• Assess clinical needs and help coordinate safe, seamless transitions to home
• Collaborate with physicians, referral partners, and AccentCare clinical teams
• Partner with Account Executives to provide clinical education and strengthen referral relationships

Required Qualifications

• Current, active LVN license required
• 1-3 years of clinical experience; home health strongly preferred
• Strong communication and relationship-building skills
