Under supervision, the Medication Reconciliation Pharmacist, when assigned to a specific patient population, shall be responsible for the medication reconciliation process at the patient's admission (and/or discharge if assigned) to prevent medication errors at this patient transition point. The pharmacist shall, by interviewing patients and working closely with pharmacy technicians, create a complete and accurate list of all pre-admission medications. The pharmacist shall compare compiled medication lists against provider orders to identify discrepancies and, if appropriate, make changes with provider permission.

Schedule: Varied 10-hour shifts, depending on department needs, including weekends and holidays

Hourly Salary Range: $91.06 - $106.67

Bargaining Unit: HX-UPTE

Qualifications

We're seeking a dedicated clinical professional with:

  • Pharm D. degree and current CA Pharmacy license (required).
  • ASHP-accredited residency (preferred).
  • 1-2 years inpatient pharmacy experience (required).
  • Exceptional communication, interpersonal, organizational and follow-through skills
