SUMMARY:
Supports the clinical and Medicaid Accountable Entity (AE) programs by providing care navigation for patients with behavioral health needs, substance use disorders and/or social needs that impact patient health and wellbeing. RESPONSIBILITIES:
Triages patient identified needs including social barriers to health and substance use disorder in order to coordinate and track referrals to community-based organizations (CBO) Coordinates and tracks behavioral health referrals to internal and external behavioral health clinicians and resources Maintains extensive knowledge of community-based organizations, external resource availability, and eligibility guidelines for each Facilitates transportation eligibility, scheduling, and other logistics for patients Performs triage of notifications from CBOs and alerts appropriate clinical staff Collects and updates the release of information (ROI) documentation to ensure coordination of patient care between organizations Participates in interdisciplinary care conferences with primary care providers and other care management teams to collaborate on an individualized plan of care Assists with clinical administrative support to Brown Health Medical Group Primary Care's Medicaid AE program Participates in Behavioral Health and AE team meetings Documents appropriately and in a timely manner in the electronic health record Complies with federal and local confidentiality laws, including HIPAA, ensuring patient privacy Adheres to Brown Health Medical Group Primary Care guidelines and policies for protecting patients' demographic, clinical and financial information Performs other job-related duties as assigned
MINIMUM QUALIFICATIONS: Minimum of high school diploma or equivalent, Associates Degree preferred * Additional training in the medical field with a social service background or demonstrated possession of the competencies necessary to perform the work * Experience working in a clinical setting with a focus on behavioral health or social services Pay Range: $20.96-$34.61
