Job Description
General Purpose of Job: Develops and implements physical therapy treatment plans for assigned patients; evaluates and modifies patient care treatments as appropriate. Assumes responsibility for creating experience-based service. Assists the Manager Clinical Services in the long-term planning and strategy of the Rehab department. Is responsible for the day-to-day operations of Clinical Services in the absence of the Manager Essential Duties:
- Responsible for the Operations of Clinical Services when the Manager is unavailable or not on-site. Performs Daily Rounds.
- Participates in daily safety briefing when the Manager is not available and assists in leading departmental huddles.
- Is responsible for creating a destination, experience-based service, delivers customized experiences using patient profiles
- Manages the daily Inpatient schedule and cultivates the relationship with the med-surg staff
- Assists the director with payroll
- Provides training to new and existing staff and has the role of a super-user for EPIC and the rehabilitation EMR
- Assists in the scheduling of clinical and front office staff
- Assists and participates in the PMR process and job specific competencies , gathers information and presents to director.
- Assists in updating department policies and procedures
- Attends required meetings in the absence of the director
- Completes initial evaluations and develops a treatment plan of care for assigned patients; coordinates acquisition of needed adaptive devices, durable medical equipment and orthoses.
- Provides and Documents patient treatments, initial evaluation, progress notes and discharge summaries in a timely manner and in accordance with Hospital policies and insurance requirements.
- Performs re-evaluations to assess patient progress; recommends changes to the treatment plan of care as appropriate.
- Communicates regularly with physicians, interdisciplinary team members, patient families, insurance companies and others regarding patient's treatment and progress.
- Instructs and teaches patients in safe transfers, activities of daily living, correct ambulation, therapeutic exercises and correct usage of exercise equipment; trains family members and caregivers on various therapeutic procedures and home exercises.
- Provides guidance to assigned staff. Duties include, but are not limited to, assigning and reviewing work, training, co-signing documentation. Provides supervision to PTAs and Rehab Aides.
- Schedules patient treatments/appointments along with referring patients to other hospital services
- Facilitates and or/attends staff meetings and in-service programs as required.
- Supports and serves as a role model for our mission, vision, values, and customer service initiatives. Adheres to the organization's policies & procedures, and compliance guidelines.
Job Requirements
| General Requirements |
* Valid State of Michigan Physical Therapist or Occupational Therapist * BLS certification |
| Work Experience |
* Minimum of 2 years rehab experience |
| Education |
* Master's degree from a Rehabilitation related clinical program (OT or PT) |
| Specialized Knowledge and Skills |
* Knowledge of physical therapy theory and techniques * Knowledge of accrediting and regulatory standards and hospital policies related to the provision of rehabilitation services including HIPAA confidentiality and JCAHO mandates |
