Job Summary
The RN Level II Case Manager at Prevea Health is responsible for assessing, planning, coordinating, and evaluating medical care for employees/patient, acting as a liaison between the worker/patient, employer, medical providers, and insurer to ensure appropriate treatment, facilitate a safe return to work, manage costs, and achieve Maximum Medical Improvement (MMI) for claim closure.

What you will do
  • Working closely with patients by obtaining medical information, performing comprehensive initial patient assessment and reassessment pertinent to specialty area of practice, and maintains patient charts/records.
  • Review medical records, assess patient needs, and develop comprehensive, goal-oriented care plans.
  • Follow nursing process of assessment, planning, intervention, and evaluation. Communicates information with patients, physician, advanced practice providers and staff to ensure quality and continuity of care.
  • Coordinates care with treating healthcare provider by noting their instructions/orders and makes necessary arrangements to ensure that they are carried out.
  • Communicates relevant clinical information to the care team, physicians, and/or advanced practice providers to optimize patient outcomes and coordinate benefits.
  • Manages patient claim concerns by actively seeking information to understand patient circumstances, problems, expectations, and needs.
  • Facilitates patient's access to appropriate level of care by assessing needs and offering clinical options using medial judgement within scope of licensure. May also refer to health care providers, services, and community resources

Education Qualifications
  • Graduation from an accredited and approved Nursing program Required
  • Bachelor's Degree in Nursing Preferred
Experience Qualifications
  • 3-5 years working experience as a Registered Nurse. Preferred

  • Knowledge, skills and ability to communicate effectively and professionally with a diverse number of internal and external customers
  • Ability to apply administrative principals and methods to include team planning, support, organization, and conflict resolution.
  • Ability to acquire and synthesize new information with a high level of detail
  • Ability to prioritize and respond to multiple interruptions
  • Strong analytical and problem solving skills
  • Strong working knowledge of Windows based computer applications.
  • Skilled in establishing and maintaining effective working relationships with patients, medical staff, and the public
  • Excellent interpersonal skills and behaviors
  • Knowledge of common safety hazards and precautions to establish a safe work environment
  • Competent in patient education/teaching
  • Ability to assist providers with exams, tests & treatments
  • Strong patient assessment skills in person and on the phone in a high-volume, fast-paced environment
  • Specialty skills as needed per department. Examples: FMLA, workers compensation, short-term disability case management.
Licenses and Certifications
  • Current license as Registered Nurse in the state of Wisconsin
    Upon Hire Required
  • Certification in Case Management and/or Certification in Occupational health Nursing and/or experience with FMLA and Workers Compensation Preferred
  • CPR certification- Basic Life Support for Healthcare Providers certification from an approved program. Required
