Job Summary
Providing exceptional care for our patients as well as recognizing the value of your co-workers is the expectation of all members of Prevea Clinic. What you will do



  • Screens and assesses the patient's learning needs and monitors and reassesses progress in a collaborative and ongoing manner.
  • Conduct both group and 1:1 educational sessions for patients and their families or support system using interactive, collaborative, skill-based training methods for the pediatric through geriatric population.
  • Provide clinical expertise and education to patients as part of the multidisciplinary education model used for these types of patients.
  • Assist patients in developing educational and behavior change goals.
  • Evaluate effectiveness of education provided by measuring attainment of learning objectives.
  • Assist patients in developing an individualized follow-up plan.
  • Document education provided through the use of electronic medical record.
  • Communicate and collaborate with other department staff and/or providers regarding patient care when appropriate.
  • Refer patients to other services when patient needs are outside of scope of practice.
  • Collaborate with other staff in development of educational training materials.
  • Participate in at least one annual continuous quality improvement process to identify and address areas for improvement.
  • Assess own strengths and weaknesses and develops a plan for professional growth.
  • Assist with all other aspects of provision of care and services within a growing Endocrinology, Diabetes, and Nutrition Services practice
  • Give patients telephone advice using professional judgment within the scope of license.
  • Serve as nutrition resource to providers, coworkers, and community.
  • Communicate with hospitals and other offices as needed and as directed by providers.
  • Adheres to established guidelines for clinical nutrition productivity
  • Utilizes appropriate functions of computer systems.
  • Willingly accepts other duties as assigned by the supervisor/coordinator


Education Qualifications



  • Bachelor's Degree Bachelor of Science Degree in Nutrition/Dietetics Required


Skills and Abilities



  • Thorough knowledge of clinical and therapeutic nutrition.
  • Able to effectively lead and manage groups.
  • Ability to develop a rapport with clients and their families
  • Basic computer skills for documentation of educational sessions.


Licenses and Certifications



  • Registered Dietitian through Commission on Dietetic Registration with current Wisconsin Certification or RD-eligible with expected achievement of RD status and Wisconsin Certification within 90 days of hire. within 90 Days Required
  • Current CPR Certification. Upon Hire Required
