Midwest Center for Youth and Families is seeking PRN/Per-diem Discharge Planner to join our dedicated healthcare professionals!

The Midwest Center is a respected healthcare provider, fully accredited, 74-bed private, residential treatment facility with locations in Valparaiso and Kouts, IN. We are dedicated to care of the hearts and minds of our patients with compassion and superior quality behavioral healthcare services through our full-immersion Dialectical Behavior Therapy (DBT) program for children and adolescents ages 10 - 18 who struggle with emotional, behavioral and mental health challenges. Position Summary:

The Discharge Planner is responsible for planning and coordinating activities related to discharge planning. Developing and implementing a plan of care designed to meet the patient's post hospitalization needs while ensuring continuity of care.

Job Duties/Responsibilities

*Communicate with referral sources within the first 24 hours of admission

*Identify all outside professionals currently working with the patient so appropriate discharge plans can be completed, scheduled with parents or significant family contacts

*Identify needs of referral sources and/or professional during patient's hospitalization period

*Oversee continued communication of treatment progress and discharge plans to outside interest parties and transfer requireed documents to outside agents

*Work with the Business Development Department on customer satisfaction

*Assist with chart audits & completion of medical records

*Inform patients and families of discharge planning options

*Act as an educational resource to hospital personnel - benefits, limitations

*prepare reports & attend meetings as necessary

Qualifications

Job Requirements:

1. Master's degree from an accredited school in Social Work or a clinical-related mental health field or ability to obtain within one year of hire

2. Minimum of 3 years experience in a psychiatric health care facility with direct experience in discharge planning required.

3. Valid driver's license and safe driving record required.

4. Must be at least 21 years old.
