Description

Position Summary: The Student Supports Coordinator will focus on creating and aligning interventions with student needs and behaviors, utilizing restorative and equitable practices. The coordinator will provide mental health services to children and their families using various social work methods, including individual, group and family therapy. These functions are performed under general supervision, necessitating the ability to exercise independent and sound judgment.

Essential Duties:



  • Assesses the social and emotional needs of patients (students, participants, members).
  • Carry a caseload of 20-25 students
  • Run Social Emotional Groups throughout the school year.
  • Hold parent workshops based on community needs.
  • Provide professional development training to staff and teachers.
  • Provide direct counseling/social services to students referred by self, faculty, administration and other school ancillary staff.
  • Coordinate social work services and patient referrals between the program and other agencies providing social services in both the school and local community.
  • Update and maintain case records by tracking and following up on all referrals made in and out of school. Establish referral network/linkages with outside agencies for both students and parents.
  • Participate and occasionally present at teacher meetings, assemblies, etc.
  • Serves as an advocate for patients' rights within the school and serves as a mediator between parents, child and school.
  • Remain available for crisis intervention as the need arises.
  • Maintain a school-wide behavior policy and tracking system
  • Assign interventions based on data from the behavior tracking system
  • Create a behavior plan for Tier 2 and Tier 3 students
  • Progress monitor interventions and student's behavior plans
  • Work with Youth Advocates and school mental health team to address students' needs
  • Facilitate Tier 1, 2, and 3 restorative interventions, including individual and social skills groups
  • Work with Parent Coordinator to connect students and families to appropriate resources
  • Actively participate in Attendance and Wellness Team meetings
  • Regularly conduct outreach to parents and guardians regarding behavior
  • Ensures that all patients (students, participants, members), visitors, guests, as well as coworkers, receive personalized, prompt attention and are treated with receptiveness, dignity and respect, no matter how limited the contact. Participates with interdisciplinary team members in developing and implementing program policies and procedures and preventing crises.
  • Performs other related duties, as required.


Minimum Qualifications:



  • Master's degree in social work or related field required
  • Bilingual (Spanish/English) preferred.

  • Ability to engage and diffuse crisis situations
  • Basic knowledge of DSM-IV, mental status and psychosocial assessments preferred.
