DESCRIPTION OF WORK

As an Aging Case Aide 2, you will be responsible for a variety of administrative support and care management activities, ensuring staff are engaged in providing assistance to consumers of long-term care in the community and in facilities. Work involves supporting the Care Management Unit by entering data, maintaining the file system, and obtaining needed information from consumers and caregivers. You will have the opportunity to accompany care managers on home visits and maintain constant communication with supervisors to provide feedback regarding areas of need throughout the unit. Additional responsibilities include:

  • Documenting activities and services provided to consumers and caregivers
  • Assigning new referrals to care managers
  • Preparing letters for physicians regarding consumer needs
  • Providing phone coverage for the unit
  • Coordinating meetings, taking notes, and tracking assignments

Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with a 30-minute lunch.
  • Telework: You will not have the option to telework in this position.
  • Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • One year as an Aging Case Aide 1; or
  • Two years of public contact work, including one year in a human services program; or
  • Any equivalent combination of experience and training.

  • Hiring preference for this vacancy may be given to candidates who live within Beaver County. If no eligible candidates who live within Beaver County apply for this position, candidates who reside in other counties may be considered.
  • You must be able to perform essential job functions.
