Position: Clinical Program Coordinator, Behavioral Health Population Health (Living Well at Home) Location: Boston, MA Schedule: 40 hours per week POSITION SUMMARY: The Living Well at Home Clinical Program Coordinator provides intensive clinical case management services to female-identifying clients and general program oversight on-site at the YWCA transitional housing program in Cambridge. The transitional housing clinician supports these high-risk clients with behavioral health support, and coordination of care and permanent housing search, offering a unique combination of housing case management and clinical intervention. This role is an integral member of the LWAH behavioral health team, and is responsible for maintaining strong relationships with key community partners in order to house vulnerable clients in the greater Boston community. The clinical program coordinator will also provide supervision to social work interns. JOB RESPONSIBILITIES:
- Manage clinical needs of complex case load of clients with serious mental illness and/or substance use disorders in a congregate living setting including weekly client check-ins, facilitation of groups as indicated, and addressing acute behavioral health crises
- Address client behavioral health crises by providing crisis intervention, assessment and triage, de-escalation and resident mediation as indicated, utilizing health system and community based resources
- Support client driven case management goals by referring clients to ongoing mental health treatment, substance use treatment, and completing housing applications
- Complete clinical documentation, assessments, and service plans in a timely manner
- Provide supervision to BSW and MSW interns
- Manage a crucial partnership with the YWCA and Cambridge Housing Authority by representing the health system and LWAH program in a professional manner
- Actively participate in all scheduled meetings, including team meetings, group supervision, and individual supervision
- Using a trauma and harm-reduction informed lens, maintain appropriate boundaries with clients and staff
- Accompany clients to apartment viewings, lease signings, medical and mental health appointments as indicated
- Participate in ongoing transitional housing program evaluation and development
- Provide stabilization support to clients who have exited the program for permanent housing through home visits and phone calls.

- Master's degree in social work or mental health counseling required
- Experience supervising trainees/students required
PREFERRED EDUCATION AND EXPERIENCE:
- 5+ years working with patient populations impacted by serious mental illness and substance use disorder
- Experience working in a congregate living setting and/or homeless services preferred
- Experience working in an independent role preferred
- Lived experience is valued
CERTIFICATIONS, LICENSES, REGISTRATIONS REQUIRED:
- Requires a Massachusetts license as an independently licensed mental health professional such as Licensed Independent Clinical Social Worker (LICSW) or Licensed Mental Health Counselor (LMHC)
KNOWLEDGE, SKILLS & ABILITIES (KSAs):
- Ability to work with a diverse and highly impacted client population using a person-centered approach
- Comprehensive knowledge of behavioral health and housing resources and systems, specifically in Greater Boston Area
- Skilled at engaging difficult to engage patients-build rapport, trust and utilizing de-escalation skills
- Ability to work in a fast paced environment, inclusive of organizational skills, time management skills, critical thinking, and problem solving abilities
- Strong interpersonal skills and ability to communicate in a courteous, pleasant and professional manner with healthcare providers, patients and families, general public, staff members, external agencies, and other organizational leaders.
- Ability to work independently and to make decisions based on established health system policies and procedures as well as the NASW or professional organization code of ethics
- Proficient oral and written English communication skills. Bi-lingual and/or bi-cultural candidates preferred
- Intermediate Microsoft Office suite experience (i.e. MS Word, Excel, Access, Outlook). Experience using an electronic health record preferred
