Job Summary:

The Case Manager (CM) is responsible for assisting and advocating for Transitional Aged Youth (TAY) who need access to resources in the community. The Case Manager conducts outreach services in order to assist TAY with identifying potential barriers to achieving stability, as well as assisting them in removing these barriers when possible. These services include but are not limited to, linkage to mental health services, medical care, educational services, prevocational services, vocational services, rehabilitative services, and other community services. The Case Manager also assists clients in reaching their goals by providing rehabilitative, collateral, and consultation services.

An important component of the role includes engaging youth and building rapport to improve their attendance and participation in the TAY Drop-in Center. The CM must be comfortable working with TAY youth from many different backgrounds and situations. This position requires a strengths-based perspective, creativity, flexibility, and a positive attitude. The CM must work well independently, be able to prioritize situations, and practice good judgment when managing crisis situations. The CM must work well under pressure and be able to work collaboratively.

Requirements:

  • Minimum of a Bachelor's degree (BA) in psychology, social work, or related field, together with at least six (6) months of experience working with children and families.
  • or Bachelor's degree (BA) in a non-related field, together with at least one (1) year of experience working with children and families.
  • or high school diploma and two (2) years of experience working with children and families.
  • or high school diploma and 1 year of experience working with children and families in an Intensive Services Program: Wraparound, IFCCS, FSP, or ITFC.
  • Bilingual Spanish preferred (verbal only).
  • Valid California driver's license; current automobile insurance; have and maintain a clean driving record acceptable to the organization's insurance company; have immediate access to his or her vehicle during work hours.

Position Location: Lancaster, CA

Salary Range: $23.39 - $33.77 per hour

Additional $1.00 per hour bilingual Spanish differential.
