Position Summary:



  • Maintains appropriate paperwork such as patients consents and other relevant documentation. Cleans and maintains inventory documentation for equipment by following Care at Home procedures and guidelines.
  • Outreaches and screens patients using set criteria for admission. Performs intake and documents necessary demographic and insurance information. Contacts patients to ensure program adherence.
  • Schedule and coordinates home visits for patients for equipment installation, education, and virtual care.
  • Tracks and assigned monitoring equipment to patients. Coordinates pick up and tracks/reports lost or damaged equipment.
  • Escalates equipment/application issues to supervisor and follows appropriate downtime procedures.
  • Submits mileage and reimbursement requests timely and according to Care at Home policies.


Work location: Hybrid - Holabird Office & in-field in patients' homes

Hours: Monday through Friday/ 8:30 - 5:00. No evenings, weekends, or on-call.

Knowledge, Skills, or Abilities: Understanding of medical terminology and electronic medical record documentation. Experience with data entry and medical equipment for recording patient vitals. Ability to communicate clearly to colleagues, team members, patients, and caregivers. Ability to perform home visits to patients and bring/install appropriate monitoring technology/equipment.

Education: High School Diploma or equivalent.

Department Preferred Licensure, Certification, Registration



  • Successful completion of an accredited medical assistant training program with active certification.
  • Must have active Maryland Driver's License and reliable personal vehicle.
  • Must have current CPR certification (AHA, Redcross, Military Training Network)


Experience
3 years in health care related field, home health or geriatric care experience preferred.

Specific systems, applications, projects on which the position will be responsible to develop or maintain:
Epic, MS Office documents including excel or access, RPM applications

Salary Range: Minimum $18.03/hour - Maximum $29.72/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.

In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
