Job Summary

Make a meaningful impact in your community-join Acentra Health as a Clinical Evaluator (PASRR) in Shawnee, Kansas. In this hybrid role, you will conduct in-person assessments in nursing homes, hospitals, and community-based settings throughout Shawnee and its surrounding areas.

As a Clinical Evaluator, you will perform face-to-face PASRR Level II assessments to determine appropriate placements and next levels of care for individuals in need. These evaluations are crucial in determining the next level of care, improving patient well-being, and supporting better health outcomes. This PRN hybrid position offers the flexibility of working from your home office, combined with local travel. All travel expenses-including mileage and parking are covered and reimbursed by Acentra Health. In addition, Acentra provides all necessary technology and equipment for your home office, including a laptop for use in the field.

Job Responsibilities

  • As a Clinical Evaluator (PASRR), you will conduct accurate screenings and evaluations, developing informed level-of-care recommendations based on your clinical assessments.
  • Conduct assessments in various environments, including nursing homes, hospitals, and community settings, to understand and address the diverse needs of those we serve.
  • Adhere to the state's best standards of mental health practices, intellectual and developmental disabilities, geriatrics, and other conditions requiring nursing care services and supports.
  • Ensure compliance with contractual and regulatory standards while developing level of care recommendations
  • Utilize independent judgment, deep clinical expertise, exceptional communication skills, and problem-solving abilities to navigate complex situations, ensuring the best outcomes for members, providers, and clients.
  • Foster effective collaboration with a diverse range of healthcare professionals, patients, families, and support networks while monitoring personal metrics to ensure compliance with turnaround time requirements.
  • Leverage technology by utilizing the state's electronic system and our proprietary platforms to efficiently manage and track all assessment activities.
  • Stay informed on current literature, regulations, and research pertaining to long-term care, mental illness, medical conditions, and geriatrics.
  • Ensure compliance with necessary workflows to maintain quality and efficiency.
  • Efficiently prioritize and execute tasks with attention to detail.

Qualifications

Required Qualifications, Skills, Knowledge and Experience

  • Active, unrestricted licensure in the state of Kansas in one of the following fields: Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), Licensed Marriage and Family Therapist (LMFT), Licensed Independent Clinical Social Worker (LICSW), Licensed Clinical Psychologist (LCP), Registered Nurse (RN), or another qualified Licensed Mental Health Professional (LMHP).
  • Minimum of 2 years' experience in assessing and diagnosing individuals with mental/psychiatric disorders, including expertise in complex differential diagnoses related to substance use, medical conditions, cognitive issues, and dementia.
  • Minimum 1 year of experience working with individuals with intellectual or developmental disabilities or related conditions.

  • Solid understanding of standards of practice related to Level of Care determinations and familiarity with standards pertaining to mental health, geriatrics, and other conditions that often require long-term care placement.

  • Demonstrated expertise in assessing and analyzing an individual's psychological, emotional, behavioral, and physical functioning.
  • Working knowledge and understanding of diagnosing psychiatric disorders in accordance with the most current edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM) applicable in the state. .

  • Proficiency in medical record abstraction, with solid knowledge of medical record organization, terminology, and common disease processes.
  • Excellent communication skills, with the ability to convey complex information clearly and effectively to individuals and groups through verbal, written, and electronic formats
  • Experience working with geriatric populations is preferred, particularly in long-term care, behavioral health, or community-based service settings.
  • Experience with public mental health and medical programs, including familiarity with public sector services and community resources, is preferred.
  • Skilled in using Microsoft Office Suite; Excel, Word, PowerPoint, Outlook, and Teams-as well as other web-based tools and platforms

Compensation

The compensation for this role is $41.00 to $43.00 per hour

Based on our compensation program, an applicant's position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
