The Advanced Clinical Pharmacy Specialist in Antithrombotic Management and Stewardship will be responsible for providing a full range of pharmaceutical care services to a specialized patient population within the inpatient and ambulatory care settings. The pharmacist will support the Jefferson Antithrombotic Therapy Service during direct patient care and consultative services including triage care, transition of care, medication therapy and perioperative antithrombotic management. The specialist will provide oversight of institutional antithrombotic usage, as well as antithrombotic - related transitional care services encompassing the health system emergency departments. The pharmacist will serve as a subject matter expert for antithrombotic usage and formulary maintenance and will work with interdisciplinary teams via various committee appointments and responsibilities to oversee assure optimal antithrombotic usage.

Job Description

Essential Functions

* Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.

* Assures the appropriateness of drug therapy and compliance with national quality standards related to the use of antithrombotic agents. Creates and oversees hospital guidelines and policies to assure compliance with standards.

* Assures implementation of processes to support delivery of quality Pharmacy services and medication therapy management

* Serves as liaison between the pharmacy department and other clinical departments

* Trains multidisciplinary staff as assigned in areas of expertise

* Serves on hospital committees, including but not limited to, the Cardiology Thrombosis Subcommittee of the Pharmacy and Therapeutics Committee, Hospital VTE Committee, regulatory readiness and patient safety committees

* Serve a preceptor to PGY1 pharmacy residents and pharmacy students as assigned

* Work with the clinical pharmacy department team to develop new protocols, service lines, and initiatives to further hospital goals and objectives

* Work with pharmacy operations to streamline drug delivery and safe medication usage with a focus on transitional care services, as well as the inpatient and ambulatory care settings

Competencies (Knowledge, Skills, and Abilities Required):

* Demonstrates advanced problem identification and resolution skills, an advanced therapeutic database and applies this knowledge in daily practice.

* Recognized as an authority in specialized Pharmacy practice area by members of the interdisciplinary team.

* Demonstrates leadership in formulating and achieving patient care goals and mentoring other members of the Department staff in this role

* Provides quality pharmaceutical care in accordance with standards of practice, demonstrating expertise, competency, knowledge and skills necessary to service the patients

* Promotes and assures compliance with the hospital formulary and the policies and procedures defined by the P & T Committee

* Identifies and reports sentinel/serious events, near misses, medication errors and adverse reactions in the electronic event reporting system

* Demonstrates, through interaction with medical staff, the ability to effectively change standards of prescribing formulary alternatives

Minimum Education and Experience Requirements:

Education:

* Doctor of Pharmacy

* Completion of PGY2 Pharmacy Specialty Residency PLUS 5 years of relevant experience ; OR Completion of PGY1 Pharmacy Residency PLUS 7 years of relevant experience; OR Completion of no residency PLUS 10 years of relevant experience * For pharmacists with 15 years of relevant experience, in lieu of above requirements, the pharmacy supervisor may determine on a case-by-case basis the most appropriate tier for the pharmacist taking into account past involvement at institution, local, state, and national level.

Experience:

* Pharmacist License in the State of Pennsylvania

* Board certification from Board of Pharmacy Specialties. In lieu of board certification through Board of Pharmacy Specialties, specialized certification in the area of clinical practice by the pharmacist may be deemed acceptable. To be evaluated on a case-by-case basis by the pharmacy supervisor.
